REGISTRATION
To register with the practice, our receptionist will ask you to fill in a form. If you have a current NHS Medical Card, please bring this with you. If you prefer a particular doctor, eg, by name or Male/Female, we will note this and do our best to respect your choice but, not all the doctors in the practice provide all services and specific doctors may not be immediately available. On registration, you will be asked to complete a health questionnaire and you will be encouraged to have a well person appointment with the practice nurse.
SURGERY HOURS
The surgery is open Monday to Friday, 8.00am to 6.00pm and routine appointments are available Monday to Friday at various times in the morning and afternoon.
Please try to be on time for your appointment, and if the doctor is running late this is usually because of unforeseen circumstances; please be patient.
Routine appointments are for one person only, lasting ten minutes, but if you feel you require a longer appointment please inform the receptionist.
If you cannot keep an appointment, please let us know as soon as possible.
Appointments may be made with the doctor or nurse of your choice by contacting the receptionist either in person or by telephone during surgery hours.
If you have an urgent medical problem, please inform the receptionist who will arrange for you to be seen as soon as possible on that day. It would be helpful to know what degree of urgency there is, but confidentiality will be respected.
If your require a medical for work, insurance, sport, driving licence etc., please let our receptionist know to allow for a longer appointment to be arranged.
TELEPHONE ADVICE:
If you wish to speak to a doctor or nurse by telephone, please ask the receptionist who will advise you when a doctor/nurse will be available.
HOME VISITS:
Except in an emergency, please request home visits as soon as possible in the morning and before 10.00am for patients who are too ill to come to the surgery.
Please give the receptionist the patient's name, address and some details of the illness, which will be treated with strict confidentiality. This enables the doctors to decide what priorities are given to home visits.
EMERGENCIES:
If you think the emergency is life-threatening, please call 999 for an ambulance. If time permits, during surgery hours, contact the surgery on (01828) 627312 or, if out of hours, telephone NHS 24 on 08454 242424 in order that a rapid response may be provided.
For all other emergencies please telephone (01828) 627312 during surgery hours or 08454 242424 at other times.
OUT OF HOURS SERVICES
The surgery is closed from 6.00pm to 8.00am Monday to Friday and from 6.00pm Friday until 8.00am Monday, and also for Public Holidays and educational afternoons which will be advised.
If medical advice is required at, these times, please telephone NHS 24 on 08454 242424 giving your name, address and contact details. Trained staff will assess and manage the problems appropriately which may be:
a) arrange advice from a doctor or nurse.
b) invite you to attend a centre to be seen by a doctor.
c) arrange a home visit if you are too ill to visit a centre.
DISABLED ACCESS
Practice premises are adapted for patients with disabilities, including single level consulting suite, wheelchair facilities, hearing loops and large print notices.
REPEAT PRESCRIPTIONS
Please request repeat prescriptions either by handing the computer printed repeat prescription slip together with a note of the items required to the receptionist or by posting same. You should also tell the receptionist whether you wish to collect the prescription from the surgery or whether it is to be forwarded to the pharmacy of your choice. If you wish the prescription to be posted to you, please enclose a stamped addressed envelope.
Please allow 48 hours for processing.
The repeat prescription slip may also be handed in to the Coupar Angus branch of Davidsons' Chemist, but please allow 72 hours for processing.
Extra time will be required over weekends, bank holidays and public holidays.
OBTAINING TEST RESULTS
The practice has a strict policy regarding confidentiality and data protection. We will only release test results to the person to whom they relate unless that person has given prior permission for the release of results or if they are not capable of understanding the result. We may ask you questions to confirm identification.
Test results are normally available 3-4 working days after the sample is taken but in some instances this can take longer and patients will be informed accordingly.
SERVICES AVAILABLE FRQM THE PRACTICE
All G.P's practices are contracted to provide "Essential Services", that is, basic treatment of all people and chronic disease management, e.g. heart disease, high blood pressure, diabetes, chronic lung disease, asthma etc.
We also provide the following "Additional Services"
> Child Health Surveillance and routine immunisation together with the health Visiting team and/or practice nurses.
> Maternity services in the ante-natal and post-natal periods together with the midwives from Perth Royal Infirmary.
> Contraceptive services which includes implant fitting services.
> Emergency contraceptive services by practice nurses or doctor.
> Cervical Smear Programme by practice nurses. A cervical smear is recommended for all women aged 20 - 60 every three years unless they have had a total hysterectomy.
> Pre-conceptual advice.
> Travel advice included recommended immunisations. Not all travel immunisations are available on the NHS, please ask our practice nurse for details.
> Freezing of warts and other small skin lesions.
We also hold contracts with NHS Tayside for the following "Enhanced Services".
> An annual flu immunisation programme to protect the elderly and those at risk.
> Regular monitoring, by blood and urine tests, for patients on a range of specialised drugs.
> Regular monitoring of patients on anti-coagulant therapy.
> Minor surgery for "lumps and bumps" etc. is offered by the doctors. They are done at mutually convenient times to patients and doctors and avoid waiting list times for hospital.

Suzanne and Deirdre are responsible for Well Person clinics, contraceptive services including emergency contraception, asthma clinics, blood pressure clinics, diabetic clinics, heart disease clinics and stroke clinics. The health visitors offer health advice and support, mainly to families with young children. This includes help with parenting and behaviour management and also support to women with post-natal depression. They see children for development checks and immunisations at the medical centre on Tuesday afternoons.

COMMUNITY MIDWIFE
Midwives provide care for expectant and new mothers in co-operation with the
doctors. Ante-natal clinic appointments can be made by telephoning the

PRACTICE PHARMACIST
The Pharmacist, Robert Lindsay, works with the Doctors and Nurses to ensure that patients get the maximum benefit from their medicines. The Pharmacist is available on a part time basis and can be contacted through the surgery telephone number.
DIETICIAN
The dietician attends the health centre once a month to provide nutritional care to individuals referred to the s,ervice by their general practitioner.
PODIATRIST
The Podiatrist (Chiropodist) attends the health centre twice a week. A domiciliary service is available for the totally housebound. Patients may refer themselves.

SOCIAL WORKERS

Occupational Therapists help people with physical disabilities, with rehabilitation and to remain living in their own homes as independently as possible by

TRAINING
On occasion, nursing or medical students may attend the practice. We hope that you will co-operate with us and help the students learn about general practice. However, you will be informed of their presence in advance, and if you do not want them to be present at a consultation, your wishes will be respected. This will not affect your treatment in any way.
SUGGESTIONS OR COMPLAINTS
We realise that sometimes things do not go as smoothly as we would like.
Suggestions or complaints should be addressed to the practice manager in writing and if you wish, you may make an appointment with her. We would wish that any complaint is settled as quickly and as amicably as possible.
In the event of a written complaint being submitted, an acknowledgement will be sent within three working days and a reply would normally be given within ten working days.
LIFESTYLE INFORMATION
1. Smoking. We are keen to give all our patients who smoke as much help as possible to stop. Please ask at reception or any consultation for information and support. Smoking cessation clinics are conducted by the health visitors.
2. Healthy Eating. A healthy diet does not have to be a boring one. Health experts inform us that we are all eating too much fat, salt etc, but what exactly does this mean? Why not speak to your doctor or nurse or pick up some of our information leaflets so you can enjoy a healthier diet?
3. Walking. Even the smallest effort can improve your quality of life and make you feel healthier and livelier. Walking is a very good exercise which most people can do and doesn't need special equipment. Simply start by walking for 15-20 minutes at a brisk enough pace to get your heart beating faster three times per week regularly. When you can easily walk for an hour you are ready to move on to running, swimming or cycling. We will:
> Ensure our patients have 24 hour access to medical advice.
> Aim for you to have access to a qualified medical professional within 48 hours of your initial contact during surgery hours, or in urgent cases, the same day.
> Work in partnership with you to achieve the best medical care possible.
> Involve you and listen to your opinions and views in all respects of your medical care.
> Prevention of disease, or injury is a primary concern and the medical staff will advise and inform you of the steps you may take to promote good health and a healthy lifestyle.
Patients can request to view their own clinical records and if necessary take photocopies. Photocopying is a chargeable service.
We would ask that you:
> Let us know if you intend to cancel an appointment or are running late.
> Treat the staff with courtesy and respect, especially as the staff may have to ask some personal questions to help us to provide you with the best service.
Inform practice staff of any changes in your circumstances such as change of name, address or telephone number - if your telephone number is ex-directory, this will be respected.
ZERO TOLERANCE TO VIOLENCE
in the rare event of a patient being violent or threateningly abusive to staff or other people in the practice we may call the police and we wil! ask NHS Tayside to remove you from our list.
